    Opening — The Assumption

    Systems that ignore human error in system design will eventually fail.

    Someone made a mistake.
    Someone didn’t follow the rule.
    Someone failed.

    So we try to fix the human.

    • add friction to risky actions
    • make errors harder to execute
    • make recovery easier

    A Guardian-type system could:

    • detect unsafe conditions in real time
    • adjust the environment before failure occurs
    • reinforce boundaries dynamically
    • guide decisions without removing autonomy

    Not by controlling behavior—
    but by supporting humans when their system is degraded


    Application

    When evaluating any system, ask:

    • Does this rely on perfect behavior?
    • What happens when attention drops?
    • Can a mistake escalate quickly?
    • Is there a buffer before failure?

    If the system breaks under normal human conditions,
    it is not safe.
